Emergency and medical information will be taken from the 2011-12 School Emergency Forms if your child is already enrolled in the School. If there is information that needs to be changed, please call the School office. If your child is new to the School, you will need to mail/fax in a copy of the child’s immunization records and fill out an emergency response card on the day he/she enters camp.

NOTE: If you register for the camp on line, your registration will be deemed to include your signature on the following permission form.
*An additional Acknowledgement and Release form, with the physical signature of a custodial parent, will be required for the Trailblazers, Trailblazers Extreme and Exploring Our Neighborhood camps. A copy of that form will be included in your camp confirmation letter and will be available in the GMS office.
The deadline for summer camp registration is May 1st.
Blanket Field Trip Permission (7-14yrs. only)
By signing this registration form, I give permission for the above named child to participate in all planned field trips and activities. I also give permission for my child to be transported in the School’s bus and/or parent/staff owned vehicles. I understand the School will use the appropriate child restraint devices and abide by all the safety rules when my child is transported in a vehicle and that I will be notified of the trip in advance.
